First Visit
Patient Registration Forms
As you probably already know, the first thing you will need to do is fill out some
forms that will provide our office with information about
yourself, your health problems and any insurance information that will be used for filing your insurance claims.
Please bring a photo ID such as a drivers license or state ID card. You will also need to bring your insurance
card so that our office manager may call and verify your coverage in our office. This will allow her to
explain to you what she believes your insurance will pay and what if any you will need to pay. All
insurance vary in thier coverage of medical and chiropractic care.
Consultation
Next you will have a consultation with the doctor to discuss your health-related problems, the
history of your health concerns concerns, as well as any past treatments you may have received and the success
you received. Then the doctor will advise you of your potential treatment options. Your first visit is allows you
and the doctor time to get to know each other and determine if chiropractic care in our office will assist you in
stabalizing your condition and return you to a healthy state again and meet your goals.
Examination
After your consultation your doctor will perform a complete chiropractic examination, including
range of motion exam, postural exam,testing your reflexes, standard neurologic including palpation of the
area to determine nerve irritaion, orthopedic, and physical examinations if necessary. Nothing will be done in
our office without your consent.
Further Testing
If X-rays are required our office will recommend that they be done at the hospital and provide you with a script to
take there to have your x-rays completed. In general this will allow you coverage through your insurance benifits.
We may request that you see your medical doctor for the order if it allows you better coverage, and have the
x-rays released to us. If you have recent x-rays, we would be happy to accept and look at them. Many times
people are amazed when the doctor explains the findings of the x-rays to them because, they did not know that,
they have previous damage and degeneration.
Report of Findings
This where the doctor will tell you if he believes our office can help you. The doctor will
go over the test results and explain to you what are your problems. He will also tell you what kind of care he
believes is best for you to meet your goals and maintain your health. If for any reason the doctor does not believe
he can help your condition, he will not accept you as a patient and will refer you to the appropriate facility. So,
you will know:
- What are your problems?
- Can we help you?
- If there is any questions about cost of the care, you will be able to discuss that
with our office manager before treatment begins.
Treatment
Patients will generally be provided with their first treatment after you, and the doctor, fully understand what
needs to be done to help to improve your health. This may include spinal adjustments, therapy, acupuncture
and/or other naturopathic therapies. If at anytime you have any questions about a treatment, feel free to ask.
Wellness Program
During your treatment our staff will recommend things that you can do at home to help speed up
your recovery . If you have a condition that is painful, cold packs will generally be recomended to reduce
inflamation and swelling. Maybe special exercises will be required. Just remember that if you have any questions,
ask and we will explain it to you. Once your health problem has improved, then the doctor will recommend a plan to
assist you in maintaining your health.
